Services
Transport
The service operates Monday to Friday providing complementary transport options to help older residents, and people with disabilities to better access the services available.
This service does not require a My Aged Care assessment. For more information and to discuss if we can assist you, contact Council's Service Access team on 03 9249 4494.
Community Connections
Our Community Connections program provides a range of activities throughout Brimbank. The program provides an opportunity for clients to attend and participate in social interactions which are conducted away from the client's home and in, or from, a fixed base facility or community based settings.
Suitability to attend specific programs will be determined by the client's specific individual needs and transport requirements.
