This award recognises those who demonstrate exceptional commitment to fostering connections, promoting inclusivity, and empowering others to participate in community life. Whether through organising events, facilitating dialogue or implementing initiatives that bring people together, we're looking for shining examples of community engagement.

Completing the online nomination form will take approximately 10-15 minutes. Please note that each question requires a minimum response of 150 words, unless otherwise indicated.
Before you begin, make sure you have the following information on hand, as the form cannot be saved for later completion:
- Contact details for the individual, group, or organisation you are nominating.
- Contact details for your nomination supporter or referee, along with a 200-word support statement.
- Any relevant attachments, such as media coverage, photographs, references, etc., to support your nomination.
